If you can't really tell he's a doctor, and that grin is on a facemask, it's not his real mouth. He speaks in reverse.

I was going to colour this with regular colour when i wanted to try some darker shading and then i realized it looked better like this. I always love pictures in this style, never have been able to pull it off well though. I might try colouring it another way sometime though.
