But let me just say a few simple things:

• They are a mafia, but this is not the entire mafia

• The girl herself, Annalisa, isn't really part of it.

• Anything with a (?) might be changed later

So, yes, the boys(and several other boys) are an Italian mafia, BUT they're not the only part of said mafia...
Actually, they're really only one part of it, the ENTIRE mafia is a gigantic international mafia, with a whole load of mafias around the world, they're only the Italian part of it.
Marco is the leader of the Italian mafia(he decides on every action the mafia has to make), every leader of the mafia has to have something to show their leadership, and in Marco's case, it's his hat(apparently, he's the only one who's allowed to wear hats in the Italian mafia, because "he said so and will "fire" anyone," if you know what I mean)
Fino is the youngest of the mafia, he's also the messenger(deals with all jobs related to outside the mafia), and Gervasio is Marco's right-hand man/second-in-command/whatever floats your boat(there's actually a close tie between them, but that'll be explained later)
