Its a sketch, you can even see the pre-sketch in the top right hand corner. This isn't yaoi, I just got curious, I mean are the teams within the Akatsuki always traveling together? What if they went to an inn or ryokan and there was only one room left? So Itachi and Kisame are sharing a bed, somehow I don't think that they would be bothered by it. They probably wouldn't care. Sleep is sleep. I hate drawing beds, so the sketch comes out way flat and the distance between the pillow and Itachi is way far because he's using Kisame as a pillow. Shark skin is smooth, isn't it? Kisame actually doesn't have any eyelids, I noticed that now, I should have paid better attention to his eyes but oh well. I know this sucks, but its cute right? I will redraw it in the future.
