Quite the predicament. To prevent it from happening AGAIN, I'd say wait until all the episodes are aired in Japan, and THEN have them available on the internet.
All their hard work...and then it's gets leaked on the internet...
I don't have any reason to suspect that MacDonald is lying (despite the fact that sometimes ANN seems a bit too uppity for their own good). I wouldn't be totally surprised if someone actually did completely undermine their security features, though they should have been certain that all the protocols were in order before receiving the episode.
The fact is, they've accepted responsibility for the matter and that's about all they can do at this point. Since they've pulled their simlucasts indefinitely, it's now become more of a case of "one ruining things for everyone" than "lolkarma".

I wanted to wait a bit before jumping the gun and going "lololololol ANN"; if they're telling the truth, then the leak is a bit more complicated than the rumors had initially stated, but they still admit fault. Really, it's just unfortunate that this happened. The ANN model was a tad too pricey for me (especially for what is initially offered), but the more concurrent streaming, the better.
As much as people want to blame the fans who found this and leaked it early, I cant. At least, I cant blame the fans completely. This exact same thing happened with One Piece and Funimation. The people involved should have known better before the One Piece leak to do what they did. Now after all of that happened, and how much it must have been talked about(especially at ANN), dont you think its a bit ironic that ANN made the exact same mistake? I do. Now they are going to go and blame fans for being fans, it makes me laugh. If someone told me I could get new episodes of NCIS or Entourage before they aired, because of a TV Network's mistake, Id take advantage of it.
So now the debate will rage on again, when this is a mistake that should never have been made. Its gullibility on the part of ANN and the OreIto staff in Japan to think that it wouldnt happen. Im sure SOMEONE had to mention this is the process of setting this up, right?
If it's the group I think it is (Horriblesubs), they're doing it as an act of trolling. They've done it before and I'm pretty sure the whole idea is to ruin it for everyone.
Once again, I'm going to suspect lax security on ANN's part for this one, though I don't have much information on the case at hand. All I know is the FUNi incidents could have easily been avoided with a little more foresight on the company's part. I'm willing to be a little more forgiving in this case though, since ANN is newer to the streaming thing than FUNi was. Considering they reported on that incident, though, you'd think they would have seen this coming.

@Ace:
I couldnt agree more on the ANN should have seen this coming. They talked about the One Piece/Funi thing at length. How can they be that forgetful?
