Yeah, my brother has a teacher that most people referred to as "teacher" or "b*tch" (the second one wasn't to her face of course) It's already kinda happening in High School. Most people just drop the Mister or Missus from the name and just refer to them by last name. (For example my teacher Mr. Framke we just say Framke to get his attention)
I guess it's 'cause parents (and teachers) say it's polite. Like I always get the lecture from a teacher that it's proper to say Mr/Mrs So and so instead of their first name or something.
Yeah I guess :) I couldn't really find a way how to word it, and I dunno. I think after someone finally tells him is Mad Hattress, he'll start saying Miss Mad Hattress

Yeah, especially right now since he doesn't know how old he is. And when I was a kid I added Mister and Miss to people's names a lot.

I think your character would specifically use the nicknames because of how the Dormouse is usually tired.
