I am American as well so I couldn't tell you absolutely for sure, but as far as I know the word manga-ka breaks down into 1: the medium of the author, this case Japanese comics and 2: does designate expertise and authorship. As for having to be Japanese, I'm sure most manga-ka are, but I don't see why they would HAVE to be Japanese.
I'll tell you what though, Nightbeck on this site, is a friend of mine, a big otaku, and she actually works as a Japanese translator and has studied in Japan. Whenever I catch her online again I'll ask her what she knows of the word manga-ka. She is the most knowledgeable person I can think of to ask.
