I'm that way too. I find that if I'm having a hard time saying something, writing it down first and then reading it to whomever I need to speak with helps make it easier. You have to be brave and remind yourself you're doing it to benefit not just you, but the other person(s) involved.

I am sure that it is hard for you to leave him, but it can not also continue that way.
You want one day to start of a family and live that life, right? Nothing will come good if it continues like that..

You will need to gather up courage and talk with him about it and how you feel. If he neglects you then as well, then I only recommend one thing and you know what that is.
