Anyway, I've wanted to make this for a long time, ever since Kelsey started the challenge, and I made some sketches of the image but I'll update the description tomorrow since now I'm in a hurry to upload this to fit within the time limit XD
Edit: phew, I finally have a moment to breathe after a hectic day, so time to write the description XD Basically, this idea was vague at the beginning, I didn't know what I really wanted to do but I knew I wanted to try a more gory approach; this is when I thought of the teeth thing, after realizing that more "realistic" and more subtle gore makes people more squeamish than, say, chopping off an arm or something (come on, I'm not the only one desensitized of movie violence). For instance, lots are afraid of going to the dentist, but I doubt there are that many fearing the ghost of Jigsaw will jump them in an alley and make them cut their foot off or something.
Aanyway, here I go rambling again. I promised the original drawings, which may be nightmare fuel, depending on your threshold of freaky images: the "face" and the girl. The arrows you see are how the light is supposed to be, I drew it during an exam and added a lot of scheme-like elements to make it look all important and professional, because I know exactly what I'm doing. And the gums are directly from the Saw III poster below, I tried painting them but it ended in fail so I stuck with stock (har har).
Oh, and this is quite important: I think this song goes perfectly with the card, atmosphere-wise c:
